Job summary

FIDRNOCT is seeking an Occupational Therapist to evaluate and provide or supervise home-based therapy, and to coordinate all components of patient home care.

The Home Care Case Manager role requires in-depth documentation, adherence to physician orders, and collaboration with the health care team in Greater Cincinnati, OH. A Bachelor's in OT and Ohio licensure are required, with CPR certification preferred.

Qualifications

• Bachelor's degree from an accredited Occupational Therapy program.
• Current Ohio licensure to practice Occupational Therapy.
• CPR Certification.

Responsibilities

• Evaluate and directly provide or supervise home-based occupational therapy services.
• Coordinate all components of the patient’s home care services as Home Care Case Manager.
• Document care and report to supervisor and care team.
• Follow physician orders and comply with policies and conditions of participation.
