FIDRNOCT is seeking an Occupational Therapist to evaluate and provide or supervise home-based therapy, and to coordinate all components of patient home care.
The Home Care Case Manager role requires in-depth documentation, adherence to physician orders, and collaboration with the health care team in Greater Cincinnati, OH. A Bachelor's in OT and Ohio licensure are required, with CPR certification preferred.
The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences. When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services. The Occupational Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.
Preferred experience: One year clinical experience in acute care setting preferred
