Job summary

Abode Care Partners is seeking a compassionate Nurse Practitioner to provide high-quality primary care to seniors and patients with complex medical needs in home settings across North Carolina.

You will conduct in-home assessments, develop personalized care plans, and collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes. Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• Master’s degree in Nursing (MSN).
• Five+ years nursing experience; at least two years NP experience preferred.
• Strong interpersonal communication skills—verbal and written.
• Excellent time management with accountability for daily workflow.
• Proficiency with medical instruments and equipment.
• Knowledge of HIPAA and OSHA regulations.
• Experience with computer-based data management and EMR systems.
• Valid driver’s license with automobile insurance.
• Ability to engage in moderate physical activity as required.

Responsibilities

• Performs evidence-based medical care by planning, directing, and evaluating patient medical needs.
• Examines and treats chronic and urgent illnesses, minor injuries, and other conditions.
• Orders and interprets labs and diagnostics as needed.
• Collaborates with caregivers and healthcare professionals to maintain a comprehensive care plan.
• Provides referrals to other practitioners when necessary.
• Maintains up-to-date patient medical records.
