Job summary

HarmonyCares is seeking a Nurse Practitioner to perform in-home risk assessments and deliver comprehensive primary care in home settings. You will conduct examinations, document visits per NCQA standards, and coordinate with physicians, nurses, and caregivers to ensure optimal patient outcomes.

Required are NP licensure, board certification, BLS, and strong EHR skills. This role involves travel within service areas and adherence to safety, confidentiality, and professional development

Qualifications

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and clean driver’s license.
• Outstanding EHR skills.

Responsibilities

• Conduct in-home health risk assessments to identify active and chronic conditions and determine all patient needs.
• Takes history, examines, and determines diagnoses.
• Provides written documentation of patient visits per NCQA standards.
• Takes patient vital signs and initiates necessary referrals.
• Places case management referrals and communicates with PCP as needed.
• Communicates with patients, caregivers, agency nurses, other providers and vendors to ensure proper diagnosis.
• Performs clinical duties while observing OSHA Universal Precautions and maintains confidentiality.
• Attends required meetings, participates in committees, and engages in ongoing professional development.
