HarmonyCares is seeking a Nurse Practitioner to perform in-home risk assessments and telehealth visits within the scope of practice delegated by the Collaborative Physician. You will deliver comprehensive evaluations, document findings, and coordinate with a care team to support high-quality home-based care.

The role requires active NP licensure, board certification, BLS, and strong EHR skills, with mobility to visit patients across service areas. Commitment to NCQA standards is essential.

Qualifications

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP or NCCPA.
• Active BLS certification.
• Enrollment in Medicare/Medicaid programs.
• Valid driver’s license with good driving record.
• Outstanding EHR skills.

Responsabilités

• Conduct in-home health risk assessments to identify active and chronic conditions.
• Take history, perform exams, determine diagnoses.
• Document visits in writing per NCQA standards.
• Take patient vitals and arrange case management referrals.
• Coordinate with PCPs, caregivers, nurses and vendors as needed.
• Maintain patient confidentiality and OSHA compliance.
