HarmonyCares is seeking a Nurse Practitioner to deliver in-home primary care and conduct annual risk assessments for patients with complex needs. The NP will work within a collaborative physician-driven model and may use telehealth as needed.

You will perform histories, examinations, and documentation in line with NCQA standards, coordinating with physicians, nurses, and caregivers to ensure integrated care.

Qualificações

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP, or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and unencumbered driver's license with good driving record.
• Outstanding EHR skills.

Responsabilidades

• Conduct comprehensive in-home health risk assessments to identify active and chronic diseases.
• Takes history, examines, determines diagnoses.
• Provides written documentation of patient visits per NCQA standards.
• Takes patient vital signs when needed.
• Places case management referrals and communicates with PCP as necessary.
• Communicates with patients, caregivers, nurses, and other providers to assure proper diagnosis.
• Maintains patient confidentiality and observes OSHA precautions.
• Participates in meetings and professional development as requested.
