HarmonyCares is seeking a Nurse Practitioner to perform in-home health risk assessments and telehealth visits as part of a patient-centered, team-based model. You will determine diagnoses, document visits, and coordinate with primary care physicians to ensure high-quality care in patients’ homes.

Qualified candidates hold an active NP license with board certification, and possess strong EHR skills. This full-time role requires flexibility for in-home visits across service areas and adherence to

Competenze

• Active, unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP or NCCPA.
• Active BLS certification.
• Current enrollment in Medicare/Medicaid programs.
• Valid driver's license with good driving record.
• Outstanding EHR skills.

Mansioni

• Conduct in-home health risk assessments and identify active and chronic conditions.
• Take patient history, perform examinations, determine diagnoses.
• Provide written documentation of visits per NCQA standards.
• Record vital signs and request/arrange further tests as needed.
• Place case management referrals and coordinate with PCPs.
