Job summary

HarmonyCares is seeking a Nurse Practitioner to perform in-home health risk assessments and telehealth visits for patients with complex healthcare needs. You will operate within the scope of practice, as delegated by the Collaborative Physician, delivering high-quality, patient-centered care.

Responsibilities include history taking, exams, diagnoses, documentation, vital signs, referrals, and coordinating with PCPs, nurses, and vendors to ensure seamless care delivery in residential settings.

Qualifications

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and unencumbered driver's license with good driving record.
• Outstanding EHR skills.

Responsibilities

• Conduct comprehensive in-home health risk assessments.
• Take patient history, examine, determine diagnoses.
• Provide written documentation of patient visit per standards.
• Take patient vital signs as necessary.
• Place case management referrals and communicate with PCP as needed.
• Coordinate with patients, caregivers, nurses, and other providers to ensure proper diagnosis.
