Abode Care Partners seeks a compassionate Nurse Practitioner to provide primary care services to seniors in their homes. You will conduct clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to improve outcomes.

Qualifications

• Active NP license in applicable state
• Current NP national board certification
• Active state DEA licensure
• MSN degree in nursing or equivalent
• 5+ years nursing experience; 2+ years NP experience preferred
• Strong interpersonal and written communication
• Excellent time management and daily workflow accountability
• EMR proficiency and basic MS Office
• Valid driver's license and auto insurance
• Ability to perform moderate physical activity during home visits

Responsibilities

• Performs clinical assessments and develops personalized care plans for homebound seniors
• Examines and treats chronic and acute illnesses during home visits and coordinates care transitions
• Orders and interprets laboratory and diagnostic testing as needed
• Collaborates with interdisciplinary teams to ensure comprehensive care
• Provides referrals to other healthcare practitioners when necessary
• Maintains up-to-date medical records and compliant documentation
