Qualifications
- Master’s degree in Nursing (MSN) and active NP license
- Minimum five years of nursing experience with at least two years as a nurse practitioner preferred
- Strong interpersonal and written communication skills
- Excellent time management and accountability for daily workflow
- Proficiency with medical instruments and equipment and HIPAA/OSHA knowledge
- Experience with EMR systems and Microsoft Office
Responsibilities
- Performs comprehensive primary care assessments and develops personalized care plans in home settings
- Treats chronic and acute conditions with evidence-based medical care
- Orders and interprets labs and diagnostics as needed
- Collaborates with interdisciplinary teams to ensure seamless care transitions
- Provides referrals to other healthcare practitioners when necessary
- Maintains accurate and up-to-date medical records
