• Make in-home nursing assessments and reassessments.
• Develop, implement, and evaluate individualized care plans with Veterans and caregivers.
• Provide direct nursing care in the home (med review, vital signs, wound care).
• Coordinate health care services across VA programs and community providers.
• Travel to Veterans' residences within the assigned area and offer flexible visit coverage.
