Job summary

U.S. Department of Veterans Affairs Sioux Falls VA Health Care System is seeking a Home Based Primary Care Nurse Case Manager to deliver comprehensive in-home nursing care to Veterans in South Dakota.

The role involves assessing complex health needs, coordinating services with VA and community partners, and supporting Veterans and caregivers to promote safety, independence, and quality of life.

Qualifications

• English language proficiency required for direct patient care.
• Current, full, active, and unrestricted RN license in the US or territory.
• Foreign nursing graduates may need degree equivalency validation.

Responsibilities

• Conduct comprehensive in-home nursing assessments and reassessments.
• Develop and evaluate individualized plans of care with Veterans and caregivers.
• Provide direct nursing care in the home within RN scope and monitor outcomes.
• Coordinate health care services across VA programs and community providers.
• Travel to Veterans' residences within the assigned geographic area.
