Job summary

InnovAge is seeking a Nurse Practitioner to provide primary care for enrolled participants, evaluating physical concerns and delivering appropriate treatment. The NP will function as an advanced practice nurse within an interdisciplinary team, with in-facility or participant-home duties as required.

Requirements include a current NP license, DEA with full prescriptive authority, and 5+ years as RN in elderly care; bilingual is preferred. Competitive benefits and steady schedule are offered.

Qualifications

• Current State issued Nurse Practitioner license.
• DEA Certification with full prescriptive authority.
• 5+ years a Registered Nurse with experience in elderly care.

Responsibilities

• Evaluates participants and documents information in participant’s chart.
• Performs physical examination and orders and interprets diagnostic tests.
• Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed.
• Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition.
• Educates staff, participants and caregivers regarding healthcare issues and treatments.
• Manages acute and long-term medical needs of participants in collaboration with other primary care providers.
• Oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings.
• Provides emotional and spiritual support to participants.
• Provides management of medical emergencies as needs dictate.
• Ensures appropriate and timely documentation of medical care in the participant’s medical record.
• Additional work functions and duties may be assigned.
