CARIN Alliance is seeking a Nurse Practitioner Trainer to support home-based primary and palliative care teams by onboarding and coaching NPs in delivering high-quality, patient-first care.
In this role, you’ll conduct onboarding and ongoing education for NPs on in-home assessments and chronic disease management, train clinicians on telehealth workflows and documentation, and mentor new clinicians to uphold safety and standards.
Competenze
- Licensed Nurse Practitioner with primary care and/or palliative care experience.
- Strong home-based assessment and chronic disease management skills.
- Proficient with telehealth workflows and compliant documentation.
- Ability to coach, mentor, and evaluate clinician competency.
Mansioni
- Conduct onboarding and ongoing education for NPs covering in-home assessments and chronic disease management.
- Train clinicians on telehealth workflows, clinical documentation, and interdisciplinary collaboration.
- Perform ride-along visits and competency skills checkoffs.
- Lead or support case reviews to strengthen clinical decision-making and care coordination.
- Partner with clinical leaders to refine protocols and support performance improvement for medically complex, home-limited patients.
- Mentor new clinicians and reinforce standards for quality and safety.
