Consumer Directed Services Inc. seeks a Nurse Practitioner to provide home-based and telehealth primary care for medically complex adults living at home.

The role emphasizes comprehensive assessments, chronic disease management, and patient-centered care across in-home and virtual settings. You will collaborate with physicians, nurses, social workers, and caregivers to develop care plans, coordinate services, and support transitions to reduce avoidable hospitalizations while upholding quality
