Job summary

UnityPoint Health in Muscatine, IA is seeking an RN Case Manager to deliver high-quality hospice care in patients' homes. You will use clinical nursing knowledge, perform assessments, teach, and implement the plan of care under physician direction to ensure positive outcomes and exceptional patient experience.

This full-time role features a Monday-Friday schedule 8am-4:30pm with rotating on-call, a salary range of $35.18 to $52.78 per hour, and a $10,000 sign-on bonus.

Qualifications

• Graduate of State Board approved program for Registered Nurses.
• One year nursing experience in a similar patient population.
• Valid RN license, including eligibility under a NLC multistate license.

Responsibilities

• Ensures compassionate, empathetic quality and safe delivery of services to patients according to care plan.
• Considers age-related needs and patient-centered approach.
• Coordinates care across the healthcare continuum with providers and care team.
• Facilitates interdisciplinary plan of care and continuity of services.
• Delivers visits according to hospice plan of care and honors patient/family wishes.
• Makes home visits and conducts ongoing interdisciplinary assessment.
• Performs initial, comprehensive, and recertification assessments.
• Performs nursing assessments at each visit.
