Job summary

UnityPoint Health is hiring an RN Case Manager to provide compassionate hospice care in patients’ homes in Iowa. You will coordinate plans of care and collaborate with physicians and the interdisciplinary team to support comfort and quality of life.

The role involves hands-on patient care and care coordination with on-call responsibilities, Monday–Friday 8:00 AM–4:30 PM. Requires RN license and at least 1 year of relevant nursing experience.

Qualifications

• Graduate of state board approved RN program.
• Licensed RN in the state of practice (NLC multistate acceptable).
• Minimum of 1 year of nursing experience with similar patient population.

Responsibilities

• Deliver in-home hospice care with compassion and dignity.
• Develop and coordinate individualized plans of care.
• Collaborate with physicians and interdisciplinary team members.
• Educate patients and families on disease progression and care plans.
• Document assessments and outcomes to ensure high-quality care.

Responsibilities

• Deliver compassionate, patient-centered hospice care through in-home visits, supporting patients and families throughout the end-of-life journey.


• Conduct comprehensive assessments, develop individualized care plans, and coordinate services across the healthcare continuum.


• Collaborate with physicians, caregivers, and interdisciplinary team members to ensure seamless, high-quality care.


• Proactively manage symptoms, identify risks, and implement interventions that promote comfort and prevent unnecessary hospitalizations.


• Educate patients, families, and caregivers on disease progression, symptom management, medications, equipment, and care plans.

Qualifications

Education:

• Graduate of state board approved program for Registered Nurses.

Experience:

• One (1) year of nursing experience caring for similar patient population as to be assigned.

Licensure/Certifications:

• Valid RN license in the state(s) of practice, including eligibility under a NLC multistate license.


• Valid driver’s license.


• Valid auto insurance.
