Qualifications

Responsibilities

• Plans, organizes and directs hospice care based on individual patient needs.
• Completes ongoing assessments of patient and family to determine hospice needs.
• Communicates with physicians regarding patient conditions and needs.
• Initiates and revises care plans as necessary.
