Job summary

A regional healthcare provider is seeking a Registered Nurse to oversee the primary caseload of hospice patients. The RN will conduct nursing assessments, develop care plans, and ensure all patient needs are met. Candidates should have a nursing degree, relevant RN license, and at least one year of nursing experience in hospice or related fields. The role requires effective communication and collaboration within an interdisciplinary team, while working in varying home environments and occasional exposure to clinical risks.

Qualifications

• Current RN license issued by the Pennsylvania State Board of Nurse Examiners required.
• Minimum of one year general nursing experience preferred.
• Understanding of hospice philosophy and principles of death and dying.

Responsibilities

• Oversee primary caseload of hospice patients and provide skilled nursing care.
• Conduct initial and comprehensive nursing assessments of patient needs.
• Collaborate to develop and implement the hospice IDT plan of care.

Skills

Experience with pain and symptom management interventions

Ability to work within an interdisciplinary setting

Effective communication skills

Education

Graduate of an accredited nursing program

Bachelor’s degree in nursing

Certified Hospice and Palliative Nurse (CHPN) certification
