Job summary

Hartford HealthCare at Home seeks a Primary Care Nurse (RN) to deliver high-quality in-home care. You will perform comprehensive assessments, develop individualized care plans, and coordinate with a multidisciplinary team to ensure optimal patient outcomes.

Autonomy is encouraged as you educate patients, promote self-advocacy, and document services with advanced technology. CT RN license and 1 year of RN experience (homecare preferred) are required along with a valid driver’s license.

Qualifications

• Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan.
• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.
