Qualifications

• Must be a Registered Nurse with valid CT RN license.
• Minimum 1 year of RN experience; homecare or hospice preferred.
• Strong communication and interpersonal skills.
• Ability to work as part of an interdisciplinary team.

Responsibilities

• Support patients, families, and loved ones through end of life journey.
• Perform a comprehensive assessment to establish goals for symptom management and end-of-life care.
• Be a member of an active interdisciplinary team addressing patients’ needs holistically.
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and families.
• Utilize state-of-the-art technology to document skilled services provided.
