Job summary

Hartford HealthCare at Home is seeking a Primary Care Nurse (RN) to provide in-home, patient-centered care across Connecticut. You will perform comprehensive assessments, coordinate with an interdisciplinary team, and educate patients to promote self-advocacy and independence.

CT RN license and at least 1 year of RN experience are required, with homecare experience preferred. A valid driver's license and computer proficiency are also essential.

Qualifications

• Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Responsibilities
