Qualifications

• Degree in Physical Therapy (Bachelors, Masters or Doctorate) from an accredited program.
• Active Florida Physical Therapist license.

Responsibilities

• Evaluate and treat residents in an assisted living or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the care team.
• Maintain accurate, timely documentation in the EMR.
