Sobe Innovative Rehab in Okeechobee, FL is seeking a licensed Physical Therapist to provide therapy services for Hospice patients in a home setting. You will evaluate and treat residents, develop treatment plans, and collaborate with physicians and families to support independence and quality of life.

Qualifications

• Active Florida Physical Therapist (PT) license required.
• Degree in Physical Therapy from an accredited program (Bachelor/Master/Doctorate).

Responsibilities

• Evaluate and treat residents in home/assisted living settings.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
