Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in a home setting in Kissimmee, FL. You will assess residents, develop personalized plans, and coordinate with the care team to support independence and quality of life for older adults.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist license required or eligible.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
