Job summary

Alignment Health is seeking a bilingual Spanish Health Coach and Educator to conduct home visits and telehealth sessions in the Care Anywhere team. You will develop wellness plans, educate patients and caregivers, and coordinate with clinicians to support higher quality of life for seniors in a remote, field-based role.

The role requires 1 year of coaching/education experience, strong communication, HIPAA knowledge, and proficiency with Microsoft Office. Mileage reimbursement is provided.

Qualifications

Minimum 1 year experience as a wellness or health coach or educator.
Bachelor's degree preferred, or two years additional experience in lieu of education.

Responsibilities

Contact patients eligible for Alignment's Care at Home program and provide wellness coaching.
Collaborate with clinicians/social workers to develop action plans.
Encourage healthier habits to improve quality of life.
Deliver high patient service and improve member retention and satisfaction.
Assist patients in accessing community resources and social services.
