Patriot Healthcare is seeking a Nurse Practitioner to deliver primary and palliative care in residential homes and living facilities across South Carolina. You will collaborate with an interdisciplinary team, develop care plans, prescribe medications, and coordinate transitions to hospice services.

• ANCC or AANP certification required.

Responsibilities

• Provide direct clinical care in home, assisted living, and independent living settings.
• Develop treatment plans and prescribe medications.
• Coordinate and provide patient-centered care and transitions to hospice services.
• Maintain thorough documentation in the EHR and participate in CE conferences.
