Job summary

Patriot Healthcare is seeking a Nurse Practitioner to deliver primary and palliative care in home and elder care settings across South Carolina. The role emphasizes direct clinical care, education, and coordinated team-based management of chronic conditions.

You'll work within an interdisciplinary team, order tests, develop treatment plans, and ensure high-quality documentation in the EHR while maintaining ongoing CE activities and travel to local facilities.

Qualifications

• Master’s degree in nursing (APRN) or postmaster certification.
• ANCC or AANP certification active in SC.
• Current South Carolina nursing licensure.
• DEA license and CPR certification required.

Responsibilities

• Provide medical care within scope of practice and perform yearly physicals.
• Educate and counsel on preventive care and chronic disease management.
• Order labs, radiology, referrals and manage test results.
• Develop treatment plans, prescribe medications and referrals.
• Coordinate patient-centered care and communicate with families and facilities.
• Document thoroughly in EHR and maintain CE attendance.
