Job summary

Patriot Healthcare in South Carolina invites Nurse Practitioners to serve local communities by providing direct clinical care in residential homes, assisted living, and independent-living facilities. We deliver Primary and Palliative Care for home-bound patients, those with transportation challenges, and the geriatric population.

Join an interdisciplinary team, manage treatment plans, order labs and referrals, provide education, and coordinate transitions to hospice when appropriate.

Qualifications

• Master’s degree in nursing or APRN required.
• ANCC or AANP certification required.
• Current SC Licensure required.
• CPR certification required.
• Current Driver’s License required.
• DEA license required.
• Minimum 2 years of experience preferred.

Responsibilities

• Provide medical care within scope of practice. Complete yearly physicals.
• Provide education and counseling on preventative care and chronic disease management.
• Order labs, radiology, referrals and review test results.
• Develop treatment plans and prescribe medications and referrals.
• Coordinate and provide patient-centered care with patients, families, and facilities.
• Maintain and attend CE and conferences.
• Provide transitional care to hospice services.
