Job summary

All Seniors Foundation is seeking a Licensed Clinical Social Worker (LCSW) to provide in-home clinical support to seniors in Los Angeles. We provide medical care, in‑home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Role Overview

As a Licensed Clinical Social Worker, you will deliver psychosocial assessment, individual therapy, and clinical case management to seniors in their homes. You will work with our interdisciplinary team to address depression, anxiety, family conflict, and end‑of‑life concerns.

Key Responsibilities

• Conduct comprehensive psychosocial assessments for seniors referred to our home‑based programs
• Provide individual psychotherapy (CBT, supportive, problem‑solving) to seniors and families
• Lead family meetings around goals of care, advance directives, and caregiver stress
• Coordinate with nurses, physicians, and case managers on integrated care plans
• Connect seniors to community resources (housing, food, legal aid, mental health)
• Document clinical encounters in EHR per CA LCSW regulations and Medicare standards

Qualifications

• Current CA LCSW license (or eligible within 90 days)
• MSW from accredited program
• 2+ years post‑licensure clinical experience; geriatric population preferred
• Comfort with home‑based and telehealth visit models
• Strong clinical documentation skills
