Job summary

Pinnacle Family Services, part of Health Connect America, is hiring for a Family Centered Treatment role serving children and families in home and community settings in North Carolina.

The position emphasizes trauma-informed, relationship-focused care. You will coordinate with medical, behavioral health, school, and community partners, develop individualized Service Plans, and maintain accurate documentation while delivering compassionate, client-centered care.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Master's in human services field + 1 year experience
• Bachelor's in human services field + 2 years experience
• Bachelor's in non-human services field + 4 years experience
• Current NC license or board-issued certification in a human services profession
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis
• Must have a valid driver’s license and access to transportation

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are — literally and figuratively
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve
