Job summary

Good Shepherd Services, Ltd. seeks a Social Worker/Case Worker for the Brooklyn Transitions Program, a borough-wide prevention initiative serving families affected by substance use and mental health challenges.

You will deliver home-based, intensive, strength-based services using the SBC framework, with supervision from the Social Work Supervisor. The role requires collaboration with ACS and other agencies, crisis intervention, and thorough documentation.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field.
• Experience in child welfare preferred.
• Bilingual English/Spanish preferred.
• Available to work at least 2 evenings per week.
• Strong organizational and counseling skills.

Responsibilities

• Provide home-based and on-site counseling for eight families.
• Participate in ACS Child Safety Conferences when referred.
• Apply SBC principles in sessions and documentation.
• Handle crises and coordinate with state agencies.
• Prepare Court reports and maintain SBC documentation.
• Collaborate with ACS, schools, clinics, and service providers.
