Job summary

Good Shepherd Services, Ltd. seeks a Case Planner for the Brooklyn Transitions Program, a borough-wide prevention initiative serving families facing substance use and/or mental health challenges.

The role is based at 503 5th Avenue, Brooklyn, NY, with 35 hours per week and a salary of $50,000–$55,000 annually based on postgraduate experience. The Case Planner delivers home-based and on-site counseling, crisis intervention, and SBC-driven case management for a designated eight-family caseload,

Qualifications

• Bachelor’s degree is required; relevant fields include Social Work, Human Services, Psychology, Sociology, Counseling, or related humanities degrees.
• Bilingual - English/Spanish preferred.
• Must have strong organizational, interpersonal, and counseling skills.
• Must be able to work at least 2 evenings per week.
• SBC trained and/or SBC certified is a plus.

Responsibilities

• Provide home-based and on-site individual, family, and group counseling, plus short-term crisis intervention, for a caseload of eight families.
• Engage families upon referral and participate in ACS Child Safety Conferences when needed.
• Implement SBC principles: assessment, outcomes, action plans, documentation and celebration.
• Handle case-related crises with professionalism and urgency, including reports to the State Central Registry.
• Conduct screenings/assessments for substance use, mental illness, and child safety and well-being.
• Integrate SBC practices into counseling sessions and ensure monthly child assessments.
• Attend required ACS and GSS meetings and trainings to represent participant needs.
• Collaborate with ACS, service providers, schools, and clinics to coordinate care.
