The Brooklyn Transitions Program by Good Shepherd Services is seeking a Social Worker/Case Worker to deliver home-based, intensive, strength-based services for families challenged by substance use or mental health issues. Work within the SBC framework to assess needs, plan, and support families in crisis.

Reports to Social Work Supervisor at 503 5th Avenue, Brooklyn, NY. Position is 35 hours per week, exempt, with a salary range of $58,000 to $72,000 annually.

Competenze

• Master's Degree Required (Must be relevant field: Psychology, Social Work, Mental Health Counselling, etc).
• LMSW eligible candidates highly encouraged to apply (exams sponsored).
• Child welfare experience preferred.
• Bilingual English/Spanish preferred.
• Strong organizational, interpersonal, and counseling skills.
• Available to work at least 2 evenings per week; trauma-informed approach encouraged.
• SBC trained and/or SBC certified is a plus.

Mansioni

• Provide home-based, on-site individual, family, and group counseling with crisis intervention.
• Engage families upon referral and participate in ACS-related meetings when needed.
• Apply Solution-Based Casework (SBC) with assessment, action plans and documentation.
• Handle crises professionally and report to State Central Registry as required.
• Carry out screenings for substance use, mental illness, and child safety.
• Ensure monthly child assessments and SBC integration in every session.
• Represent family needs at ACS and GS meetings and trainings.
