Job summary

Evergreen Life Services seeks compassionate, reliable individuals in Tennessee to welcome a person with disabilities or an elderly individual into their home as a Family-Model Provider.

Responsibilities include daily personal care, health management, and helping with independent living skills, employment support, and community integration. A private bedroom and adherence to background checks are required.

Qualifications

• Age 21+ (25+ for youth/adolescents)
• Must live in stable TN residence meeting HUD safety guidelines
• Private, available bedroom that meets placement regulations
• Financially stable to cover household needs independently
• Pass state background checks, fingerprinting, and home study by Evergreen

Responsibilities

• Daily Needs & Personal Care: assist with daily tasks and personal hygiene
• Health & Medication Management: administer medications and accompany to appointments
• Independent Living Skills: support development of daily living abilities
• Employment & Community Support: assist with ECFC program and transportation
• Companionship & Integration: provide a warm home environment and socialize with community
