Job summary

Evergreen Life Services seeks compassionate, reliable individuals to become Family-Model Providers in Tennessee. You open your home to individuals with disabilities or elderly residents, offering daily living support, meals, and companionship within a safe home setting.

Responsibilities include personal care, health management, and documenting progress while complying with Evergreen policies. Training and round-the-clock agency support are provided to ensure sustainable caregiving arrangements

Qualifications

• Must be 21+ years old (25+ for youth/adolescents under DCS).
• Must live in a stable Tennessee residence with safety compliance.
• Must have private bedroom meeting placement regs.
• Pass state background checks, fingerprinting, and home study by Evergreen.
• Driver's license, clean driving record, reliable vehicle, and auto insurance.

Responsibilities

• Provide daily personal care and meals.
• Assist with health and medication management.
• Support independent living skills and daily routines.
• Assist in Employment & Community First (ECF) program activities and transportation.
• Offer companionship and integrate residents into home and community activities.
• Maintain detailed health and progress records as required.
