Job summary

Evergreen Life Services seeks compassionate individuals and families in Tennessee to become Family-Model Providers. You open your home to provide care for individuals with disabilities or elderly adults, helping them develop daily living skills and achieve goals.

Core duties include personal care, health management, transportation to appointments, and community integration, with documentation and compliance to Evergreen's terms. Training, support, and stipends are provided.

Qualifications

• Must be at least 21 years old (25+ for youth/adolescents under DCS).
• Must live in a stable Tennessee residence meeting HUD safety guidelines.
• Private, available bedroom that meets placement regulations.
• Financial stability to meet own household needs.
• Pass state-mandated background checks, fingerprinting, and home study by Evergreen.

Responsibilities

• Provide daily personal care and hygiene assistance.
• Manage health and medications and escort to medical appointments.
• Assist with independent living skills and daily routines.
• Support employment and community activities under ECf CHOICES program.
• Offer companionship and integrate individual into home and community life.
• Maintain detailed daily records of health metrics, behavior, and milestones.
• Comply with terms in the independent contractor agreement.

Evergreen Life Services believes that community-based care is the key to helping individuals live their most abundant and independent lives. We are seeking compassionate, reliable individuals and families across Tennessee to become Family-Model Providers.

In this role, you will open your home and heart to provide care for an individual with an intellectual, developmental, or physical disability, or an elderly individual. By welcoming them into your home as a family member, you will help them develop daily life skills, encourage personal choice, and support them in achieving their goals.

Core Responsibilities

• Daily Needs & Personal Care: Provide compassionate assistance with daily tasks, including personal hygiene, eating, and meal preparation.
• Health & Medication Management: Safeguard the individual’s physical health through accurate medication administration and transporting/accompanying them to medical appointments.
• Independent Living Skills: Help individuals master daily life skills and foster their independent living capabilities.
• Employment & Community Support: Assist individuals participating in the Employment and Community First (ECF) CHOICES program with community integrated supports, transportation, and daily routines that foster consistent employment.
• Companionship & Integration: Offer a warm, inclusive home environment. Provide ongoing companionship and involve the individual in household and community social activities.
• Documentation: Keep detailed, daily records of health metrics, behavior tracking, and goal milestones as required by Evergreen and state regulations.
• Contract: Must comply with all terms and conditions specified in the independent contractor agreement.

Qualifications & Requirements

• Must be at least 21 years old (25 or older if working with youth/adolescents under DCS programs).
• Must live in a stable Tennessee residence (owned or rented) that meets all state and HUD safety guidelines.
• Must have a private, available bedroom that meets placement regulations.
• Must be financially stable and able to meet your own existing household needs independently.
• Ability to pass all state-mandated criminal background checks, fingerprinting, and a successful home study completed by Evergreen.
• Must possess a valid driver's license, clean driving record, reliable vehicle, and active auto liability insurance.
• Complete all mandatory Evergreen pre-service training, including CPR/First Aid certification.

• Room & Board Reimbursement: A designated monthly amount contributed by the individual to help cover household expenses.
• 24/7 Agency Backup: Ongoing support with access to an assigned Evergreen Program Coordinator and 24/7 on-call emergency assistance.
• Training & Respite: Continuous specialized training to meet the individual's specific needs, alongside scheduled respite relief to prevent burnout
