Job summary
Evergreen Life Services in Tennessee seeks compassionate families to become Family-Model Providers, opening your home to individuals with disabilities or aging needs. You help them build daily living skills and independence while offering a warm, supportive home environment.
Caregivers provide daily care, health support, transportation, and community engagement, with detailed documentation and adherence to Evergreen policies. A tax-exempt stipend and 24/7 agency backup accompany the role.
Qualifications
- Must be at least 21 years old (25+ for youth/adolescents).
- Live in a stable Tennessee residence meeting HUD safety guidelines.
- Private, available bedroom that meets placement regulations.
- Financial stability to meet household needs independently.
- Pass state background checks, fingerprinting, and home study by Evergreen.
- Valid driver's license, clean driving record, reliable vehicle, insurance.
- Complete Evergreen pre-service training including CPR/First Aid.
Responsibilities
- Provide daily needs and personal care with compassion and dignity.
- Manage health and medications and accompany medical appointments.
- Develop independent living skills and daily routines.
- Support Employment & Community First (ECF) program participation.
- Offer companionship, inclusivity, and community engagement.
- Maintain detailed records of health metrics, behaviors, and milestones.
- Adhere to terms in the independent contractor agreement.
