Upward Health, Inc. is seeking a Care Specialist to deliver chronic care management primarily in patients' homes and communities, with some virtual care. You will guide enrollment, coordinate with providers, and ensure adherence to care plans for high-complexity patients.

The role emphasizes relationship-building, care coordination, and education to improve outcomes and reduce hospitalizations, while supporting patients in managing diverse health needs.

Qualifications

• Minimum of 2 years of relevant work experience such as CHW, peer support, or medical assistant.
• High school diploma or GED required.
• A valid driver’s license, auto liability insurance, and reliable transportation to travel within territory.
• Experience in chronic care management or with chronically ill/elderly patients.
• Experience with EMR systems and basic computer skills.
• Experience with motivational interviewing, trauma-informed care and care coordination.
• Strong interpersonal communication to engage patients and team members.
• Ability to prioritize tasks and work independently in unstructured environments.
• Multi-lingual skills are a plus but not required.
• Prior home care experience is beneficial.

Responsabilités

• Deliver chronic care management in patients' homes and communities (90%) and virtually (10%).
• Guide enrollment, facilitate virtual appointments, and ensure adherence to care plans.
