Position

Full‑time Case Manager for Bangor/Northern ME to coordinate services for adults with developmental disabilities. The role oversees supports for individuals in their homes, at work, and with community supports.

The case manager works with families, spouses, providers, and state agencies. The position requires a self‑motivated individual comfortable working independently and able to multitask in a fast‑paced environment.

Team support is available. Candidates must own a dependable automobile with comprehensive insurance coverage. Mileage reimbursement is provided.

Responsibilities
  • Co‑ordinate services and supports for adults with developmental disabilities.
  • Conduct assessments, develop individualized care plans, and monitor progress.
  • Collaborate with family members, caregivers, service providers, and state agencies.
  • Maintain accurate case documentation and comply with program policies.
  • Advocate for client needs and promote community inclusion.
Qualifications
  • BA in psychology, human services, social work, special education, rehabilitation, or closely related field.
  • Experience working with adults with developmental disabilities preferred.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage multiple priorities.
