Qualifications

• High School Diploma and 3 years of relevant behavioral health experience, or a Bachelor's in a social sciences field.
• Understanding of mental illness, psychiatric symptoms, and impact on functioning.
• Valid driver's license, car insurance, and insurable driving record; willingness to travel.
• Reliable transportation and willingness to travel throughout the service area.

Responsibilities

• Coordinates assessments, treatment planning or crisis intervention services.
• Facilitates healthcare, behavioral health and other services and supports as part of care coordination.
• Provides training and facilitates linkages for the use of community resources.
• Monitors symptoms and service delivery to ensure client needs are met.
• Obtains or assist clients in obtaining necessary services for basic necessities and services.
• Assists clients in accessing self-help support groups and increasing social support networks.
• Provides appropriate interventions to aid clients in the restoration of daily living skills.
• Coordinates and assist in crisis management and stabilization.
• Collaborates and communicates effectively with treatment team for the coordination of care.

Education

High School Diploma
