Job summary

Community Health Alliance is seeking an experienced Case Manager (QBHS) to support children, adolescents and adults with mental health conditions by coordinating care, teaching daily living skills and fostering recovery through individualized services. This community-based role includes home visits, service coordination and collaboration with providers and families.

The ideal candidate will coordinate assessments, plan treatment and connect clients with community resources, while ensuring

Qualifications

  • Bachelor’s degree in a social sciences field or related field.
  • Understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior.
  • Knowledge of how to therapeutically engage a person with mental illness.
  • Valid driver's license, car insurance, and insurable driving record.
  • Willingness to travel throughout the assigned service area.
  • Reliable transportation.
  • Understanding of the community mental health system.

Responsibilities

  • Coordinates assessments, treatment planning or crisis intervention services.
  • Facilitates healthcare, behavioral health and other services and supports as part of care coordination.
  • Provides training and facilitates linkages for the use of community resources.
  • Monitors symptoms and service delivery to ensure client needs are met.
  • Obtains or assist clients in obtaining necessary services for basic necessities and services.
  • Assists clients in accessing self-help support groups and increasing social support networks.
  • Provides appropriate interventions to aid clients in the restoration of daily living skills.
  • Coordinates and assist in crisis management and stabilization.
  • Collaborates and communicates effectively with treatment team for the coordination of care.
