Qualifications

• Board-certified APRN with active national certification.
• Eligible for NH and VT licenses and DEA.
• Palliative care fellowship or 2+ years hospice experience.
• Certified in Advanced Practice Palliative Nursing or eligible within one year.
• Licensure as Adult NP/AGNP/Family NP in NH/VT.

Responsibilities

• Evaluate, assess and manage palliative care needs, including pain and symptom management.
• Provide education to patients and families according to plan of care.
• Collaborate with interprofessional team in home and facility settings.
• Assist with diagnosis, treatment and management of acute and chronic conditions.
• Consult with Chief Medical Officer and inform primary providers as needed.
• Educate staff and community about home-based palliative care programs.
