Job summary

Dartmouth Health Hospice seeks a Nurse Practitioner (APRN) to support comprehensive hospice services in Vermont and New Hampshire. The NP performs face-to-face assessments for hospice recertification as part of the interdisciplinary team and documents findings for eligibility decisions.

Qualified candidates must have an accredited NP program, VT or NH APRN licensure (and within 30 days of hire both), BLS within 90 days, and 2 years of experience preferred.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner program.
• Two years of experience preferred.

Responsibilities

• Performs history and physical examinations and documents findings for face-to-face hospice recertification.
• Conveys clinical findings to the certifying hospice physician for ongoing eligibility decisions.
• Collaborates with the interdisciplinary team to support recertification evaluations.
• Completes, maintains and submits accurate clinical notes and related documentation.
• Engages in ongoing professional development through seminars, conferences and continuing education.
