Job summary

HarmonyCares seeks a Family Nurse Practitioner to deliver home-based primary care to medically complex, home-limited patients. In this physician-led, interdisciplinary model, you’ll conduct in-home and telehealth visits, manage chronic conditions, coordinate with palliative and care management teams, and help reduce avoidable hospitalizations.

Qualifications

• Must be a licensed Family Nurse Practitioner in Ohio.
• Experience in home health or primary care is a plus.
