Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver compassionate, home-based primary and palliative care to medically complex patients in Queens, NY. In this full-time role, you will conduct in-home visits and telehealth consultations, perform comprehensive assessments, manage chronic conditions, and develop individualized care plans.

Qualifications

• Active Nurse Practitioner license in NY.
• Experience in primary and palliative home-based care preferred.
• Strong assessment and care-planning skills.

Responsibilities

• Conduct in-home visits and telehealth consultations.
• Assess and manage chronic conditions; develop individualized care plans.
• Coordinate interdisciplinary team to prevent hospitalizations and support patients/caregivers.
• Document care in the EHR and maintain accurate records.
