Job summary

Mountain Valley Hospice & Palliative Care in North Carolina is seeking an experienced Hospice RN for after-hours on-call, home-based. You will provide compassionate care and reassurance to patients and families, assess needs, and determine visits from your home-based space.

Qualifications

• Current RN license.
• Strong clinical assessment, communication, and critical-thinking skills.
• Ability to work independently and respond to changing patient needs.
• Hospice, home health, or related nursing experience preferred.
• Ability to remain within the assigned service area during scheduled on-call hours and travel to patients' homes when needed.

Responsibilities

• Respond to after-hours patient and family calls.
• Assess patient needs and provide nursing guidance and support.
• Make in-home patient visits when clinical needs require hands-on care.
• Collaborate with the hospice care team to ensure continuity of care.
• Provide comfort, symptom management, and compassionate support to patients and families.
