Job summary

RHA Health Services, LLC is hiring AFL Providers in Lumberton, NC to support adults and/or children with I/DD in a home setting. You will work as an independent contractor, providing compassionate care, meal support, and home upkeep while following the person-centered plan.

The role emphasizes dignity, privacy, and individualized goals, with training provided by RHA and supervision arranged with a dedicated supervisor.

Qualifications

HS Diploma/GED is required.
18 years old or older.
Documentation of home ownership: mortgage or renter's lease.
Proof of homeowner's renter insurance or personal property insurance.
Valid Driver's License.
Able to pass a criminal background check, Healthcare Registry and Urine Drug Screen for yourself and other adults in the home.
Work as an Independent Contractor.

Responsibilities

Understand the individual in the context of their personal history and goals.
Support the individual with meals, self-care, rest, education, and community activities.
Prepare, teach, and assist with food preparation and home cleaning.
Honor the choices and culture of the person supported.
Provide opportunities for social, emotional, and spiritual needs.
Assist with transportation and scheduling of medical, therapy, vocational, and recreational activities.
Ensure the home is safe and maintained.
Support through the Person-Centered Plan addressing medical, psychiatric, recreational, and emotional needs.
Attend treatment team meetings and facilitate communication about needs and progress.
Participate in supervision as outlined in the RHA supervision contract.
