Requires a BA/BSW, fluency in Russian, and 2+ years in geriatric case management. Hybrid work schedule (3 days on-site, 2 days remote) with NY borough travel.

Qualifications

• Bachelor's degree required (BA/BS/BSW).
• Fluency in Russian required.
• 2+ years of geriatric case management experience.
• Proficiency with Salesforce and Microsoft Office and electronic records.

Responsibilities

• Provide trauma-informed, patient-centered case management services.
• Assist in financial assistance, benefits applications, and referrals.
• Offer home visits and office-based support.
• Document client interactions in Salesforce and maintain records.
• Collaborate with partner agencies and organize survivor social events.
• Address crises with ethical, professional intervention.
