Job summary

Jewish Family and Children's Services (JFCS) is hiring a Care Manager for the Holocaust Survivor Programs in the San Francisco Bay Area. The role focuses on in-home assessments, care planning, and coordinating services with families and community partners.

The Care Manager will ensure documentation, eligibility support, and compliance with funders and agency requirements while promoting client safety, independence, and quality of life.

Qualifications

  • Bachelor’s degree required in Social Work, Gerontology, Human Services, Public Health or related field.
  • LCSW or higher social services licensure preferred.
  • Valid CA driver's license and reliable transportation; travel to JFCS regions/sites.

Responsibilities

  • Deliver ongoing supportive services to clients, families, and caregivers via phone, video, email, and in-person visits.
  • Conduct in-home assessments and develop individualized care plans.
  • Coordinate services and ensure timely documentation and compliance with funders and agency requirements.
  • Maintain knowledge of community resources and advocate for client needs.

Conversational Russian
MS Office
EHR/CRMS experience

Education

Bachelor’s degree in Social Work, Gerontology, Human Services, Public Health, or related field
Licensure: LCSW or higher preferred
